We offer case management for chronic conditions that do not require specialty care or advanced imaging. These include hyperthyroidism, chronic kidney disease, diabetes mellitus, and intestinal disease.

We can manage other chronic conditions including osteoarthritis, pain and asthma, but are unable to perform x-rays which should be obtained to confirm the diagnosis prior to treatment. For cases requiring x-rays, we will refer to local full-service general practices to obtain the images.

Any cases we are unable to manage, including complicated internal medicine, oncology, ophthalmology, dermatology or behavior cases, will be referred to the appropriate local specialist.
